Topical vs Intravenous Tranexamic Acid in Reducing Blood Loss After Bilateral Total Knee Arthroplasty: A Prospective Study

Abstract : Aim: In simultaneous bilateral total knee arthroplasty, is there a significant difference in efficacy between topical tranexamic acid and intravenous tranexamic acid? 70 patients scheduled for simultaneous bilateral total knee arthroplasty were randomized to administration of intravenous or topical tranexamic acid. The purpose of this study was to compare, primarily, hemostatic efficacy and safety between the two groups, per operatively, as well as clinical outcomes at follow-up. The results of this trial demonstrated that the estimated total blood loss, postoperative blood drainage, and the incidence of transfusion were significantly lower in the topical TXA group compared to the IV TXA group. Clinical outcomes favored the topical TXA group Material & Method: The study was approved by the Ethics Committee at our hospital and all the patients gave their written informed consent. We enrolled 70 patients with either rheumatoid arthritis or osteoarthritis who were to have a bilateral cemented TKA. Result: Mean total blood loss was significantly lower in the topical TXA group (543+/-264mL) compared to the IV TXA group (1039+/-483mL).
